Notice of Data Incident 04/09/2021

What Happened? On April 9, 2021, an unauthorized individual gained entry to a BASS Medical Group office and stole several items including a stack of papers containing the scheduled appointments and some related information for a limited number of BASS Medical Group patients. An investigation was undertaken to determine what may have been accessed by the intruder and what PHI may have been at issue. In addition, papers containing PHI were recovered by the Emeryville police from a suspect in the break in. BASS Medical Group determined that patient information was taken by the unauthorized individual during this access.
What Information Was Involved? The information accessed by the unauthorized individual may have contained your name, address, date of birth, phone number, and prescription, diagnosis and treatment information.
What Is BASS Medical Group Doing? Data privacy and security are among BASS Medical Group’s highest priorities, and we want to assure you that we are taking steps to minimize the risk of this kind of event from happening in the future. We have taken additional steps to fortify physical entry points to our offices. We are in contact with law enforcement and are cooperating with them in their investigation of this matter.
What You Can Do. We do not believe that these papers were of any real value to the intruder and are unaware of any misuse of the information taken by the unauthorized individual. That said, it is always a good idea to review your estimate of benefits and other health information to identify any suspicious activity.
