Several physicians in our practice have extensive training and experience in electrodiagnostic procedures including EMG/nerve conduction studies, EEG (electroencephalography), and evoked potential studies that can be used to help diagnose a neurologic condition.
Botox (Botulinum toxin) treatment
Several physicians in our practice have expertise in botulinum toxin injections for treatment of dystonia, spasticity, hemifacial spasm, blepharospasm and headaches.
Your First Visit
Please note that all appointments must be confirmed by yourself during the week prior to your appointment. This may be done by confirming at the time of your automated confirmation call by our office 2-4 days prior to your scheduled visit. If your appointment is not confirmed 24 hrs prior to the visit time, your appointment will be cancelled and another patient will recieve your appointment time. We require a credit card or advance deposit to hold/reserve a new patient appt. Your credit card will not be charged unless you do not arrive for your appointment and did not cancel the appointment at least 24 hrs in advance. Advance deposits will be refunded to you at time of your appointment. We are sorry this policy has become necessary, and apologize for the inconvenience.
Please download the “New Patient Registration and History Forms”, fill out all relevant information and bring to your first visit. The forms are expected to be completed at the start time of your appointment. You will also be provided with privacy information relating to the Health Insurance Portability and Accountability Act, passed in 1996, with a signature page to acknowledge that you have received this information.
Please plan to bring copies of all relevant previous medical evaluations, all of your current medications, insurance information, laboratory reports, x-ray reports, CAT scan reports, and MR scan reports with you for your neurologist appointment. Please download the medical records request form and fax to all previous medical providers who may have treated you for your current condition. It is important to bring copies of these records to your appointment to avoid further delay in diagnostic work-up or duplicate testing. Please plan to arrive for your appointment at least 15 minutes early to provide time to complete registration paperwork. Arriving more than 10 minutes late for a new patient visit or 5 minutes late for a follow-up visit may result in inadequate doctor evaluation time and rescheduling of your appointment.
You are expected to provide co-pays or full payment at the time of your office visit as required by your insurance plan. For your convenience, we accept cash, check, or major credit cards.
What is an EEG?
This is a test to record the electrical activity of your brain.
The tech will measure your head and then apply 24 sensors using a cool scrub and a little pressure.
The process does not hurt and it is noninvasive. You will be expected to sit still for the entire recording (40 minutes/1 hour/however long MD requested).
For a 40 minute recording please allow an hour and a half for the entire appointment.
For a 1 hour recording please allow an hour and forty five minutes for the entire appointment.
THE RECORDING IS INTERPRETED BY A NEUROLOGIST. IT MAY TAKE UP TO A WEEK TO RECEIVE YOUR RESULTS.
